• Prism Health North Texas combats HIV/AIDS in the community by improving the lives and health of individuals living with the disease and preventing its spread. Prism Health North Texas is an approved primary entry point for the Ryan White HIV continuum of care in North Texas serving 12 counties. It provides case management, outpatient medical care, medication assistance for eligible individuals, referrals to mental health and substance abuse treatment, and assistance with improving access to medical care, treatment, and assistance with improving access to medical care, treatment and support services. It also provides HIV testing, counseling and education regarding risk reduction and prevention. The HIVE provides empowerment programs and social support activities for anyone living with HIV. Prism Health North Texas works with community organizations throughout North Texas to facilitate linkage to medical care, dental services, housing, transportation, food, psycho-social counseling, substance abuse treatment, health insurance assistance, disability and financial assistance, government benefits, financial and legal planning, and caregiver support services. Staff is trained and able to work with a diverse population and those with specific needs. Interpreters are available for Spanish. Case management services are available at our locations in Oak Cliff, South Dallas, and Dallas County Health & Human Services. Home visits may be possible in certain circumstances. LifeWalk is the agency's signature fundraising event which provides program funds for the agency & LifeWalk Partner Agencies. www.lifewalk.org.
