Since opening the doors of our first community practices in 2013, we have employed 20 primary care providers and are strategically growing throughout Eastern MA. We are transforming the way that community primary care physicians are compensated; by offering a competitive structure that aligns with the quadruple aim (high quality, efficiency, patient satisfaction, reducing physician burnout).Tufts MC Community Care is here to offer you a desirable alternative for how you can practice adult primary care medicine the way you always imagined- focused on the patient, with the support of a clinical team, in a collegial, academic tied community based practice.Tufts Medical Center Community Care has practices in Framingham, Quincy and Wellesley- with our newest location in the Woburn area opening October 2017.Tufts Medical Center Community Care Highlights:Team-based model of care:- Mix of MAs, RNs & LPNs, NP/PAs to support the practice in achieving the highest quality of patient care and reducing the physician burnout rateAs a member of New England Quality Care Alliance (NEQCA), our practice receives the benefits of comprehensive support services including:- RN Care Management & Health Coaches- Pharmacy Tech Support (including a unique Prior Authorization Program)- Coding and Billing Support- Behavioral Health Provider and Social Work Support- Clinical Data Analyst - Medical Home and quality supportPrivate Practice Feel- Each location is staffed to 5-7 PCPs- Physicians are hired site specific (no rotating between different practices)- eClinical Works is utilized as the EMR platform
