- PACE is a Program of All Inclusive Care for the Elderly. It is a Medicare and Medicaid approved program to support the frail elderly that meet criteria to be in a nursing home but can live at home safely with the support of the PACE services. Other eligibility criteria include 55 years and older and must live in a PACE service area. Services provided include medical, nursing, social service, rehabilitation, recreational day care, nutrition and transportation to and from the PACE center or to medically necessary appointments. Beacon of LIFE services many areas in Monmouth County and is located in Oceanport in the Fort Monmouth area. The program is based on meeting the individual needs of an enrolled participant. Enrollment is voluntary and one may disenroll at any time. Participants may be fully liable for the costs of unauthorized or out of PACE program services.
