- We are a relocation & transition agency offering transition coordination services for people on medical assistance who live in nursing homes, hospitals, & Intermediate Care Facilities move into independent settings in the community!We believe in people over process. Our top priority is to ensure that no one is overlooked, and each client is given the opportunity to have their voice heard so they can move from dependent living situations to independent ones that work for them. Our compassion and tenacity, enables us to restore our clients hope by helping them home. Our Services: Our Relocation Specialists create a transition plan for each client by: Searching and applying for places to live Scheduling and attending housing tour options Helping apply for subsidized housing (Section 8, Section 42, HCV) Applying for financial assistance Setting up PCA or other services in new client homes Assisting in seeking for and applying for employment Arranging moving day transportation Who Qualifies: Our services are a benefit of medical assistance.
