- A Health Home is not a place, its a free service offered to members through an individual's Medicaid coverage. Health Homes provide Care Management services to help make sure everyone involved in a members care is working together and sharing information that is important in supporting a members health and recovery. A Health Home Care Manager is expected to help coordinate not just medical, emotional well being, and substance abuse services, but also social and community service needs of their members.The Health Home will assist members with:Getting members involved in activities to improve their quality of life and to keep our members healthyHelping the members learn about social and other activities in the communityHelping with a discharge plan to transition a member from the hospital back to the communityMaking sure everyone involved in a members care understands the members care plan, created with the member to help meet their goalsHelping the member obtain safe housing, legal assistance, food and other essential needsTo be eligible for Health Home Care Management individuals must be active Medicaid recipients, including those who are members of a Medicaid Managed Care Organization. Individuals who have both Medicaid and Medicare are also eligible. Individuals must also meet the following eligibility criteria:Have two or more Chronic Health Conditions (such as asthma, diabetes, heart disease, mental health conditions or substance use disorder); or Have a Serious Mental Illness or Be living with HIV/AIDSand have a need for care management.Referrals for CNYHHN, Inc.
