- M & M Home Care employs only experienced, compassionate and reliable healthcare professionals who deliver a comprehensive set of services within the patient's home. Our mission is to provide consistent, uninterrupted care with ongoing nursing oversight while building trusting relationships between the patient, their families and the M & M Home Care Team. Our Small Team approach pairs each patient with a select, consistent team of caregivers based on a variety of factors with the ultimate goal being continuity of care. This approach readily identifies changes in patient behavior and care staff are more likely to notice slight changes in a patient's health which, in turn, can prevent escalation of more serious health problems. Every patient is assessed by a Registered Nurse, prior to the Start of Care, who then creates a customized Plan of Care (POC) specific to the patient's needs. Once complete, the RN will then coordinate any additional service needs and will provide an orientation to the Care Team.
