• OpenNotes is a national initiative that urges doctors and other clinicians to offer patients ready access to their visit notes. Neither a software program nor a new technology, this cultural change in practice represents an important step in the movement toward greater transparency in health care. OpenNotes started in 2010 as an experiment with more than 100 primary care doctors from three diverse medical institutions across the United States sharing notes online with 20,000 of their patients. The results of the experiment were published in the Annals of Internal Medicine and the OpenNotes movement quickly began to spread. The research suggests that reading notes written by doctors, nurses, and other clinicians can prompt patients to be more active in their own health and health care, and greater patient engagement can contribute to better outcomes, safer care and reduced costs. In the three years since the findings were reported, the OpenNotes movement has achieved widespread adoption by major medical systems nationwide. Notes are being shared, not just in primary care, but also in specialty care, mental health, and even in the intensive care unit. More than 7 million Americans now have access to their medical notes and the number continues to grow. The goal is for OpenNotes to become the standard of care for all patients.
