- PCC is a multi-faceted Healthcare consulting firm, with many years experience in sub-acute and long term care. Our concentration is on continuity of patient care. We assist hospital Social Workers, Case Managers. Discharge Planners, Geriatric Care Managers, Community Resource Centers and patients/patients family by making the transition from the hospital to the long term care facility as smooth as possible. Please note that there is never a cost to the hospital or the family for the services that we provide. We specialize in assisting patients, families and hospitals with the placement of patients into Nursing Homes and Adult Homes. We represent an extensive network of facilities which continues to grow. We represent many facilities thus enabling us to expedite transfers from the hospital to long-term care. By utilizing our service the hospital staff can avoid lengthy and unproductive hours of work commonly associated with contacting many different facilities and placing residents; hospital length of stay for patients awaiting transfer will decrease; and patient satisfaction will increase. Our areas of expertise include out of borough, Medicaid pending, HIV, complex medical cases, young, low score, ventilator/trach to name a few.
