• Leading By Example is a mental health services agency serving the Baltimore & Harford areas that is committed to ensuring the holistic health & development of children, adults, families & communities. LBE utilizes a holistic well-being approach to produce healing & restoration for our clients. LBE provides PRP, Health Home, & TBS to over 400 clients. Psychiatric Rehabilitation Program PRP services are offered to adult & minor clients who experience difficulty independently managing their mental, emotional, & physical health needs. Services are provided in the clients home, within the community, & at the LBE Office. Minor clients receive 3-4 one hour visits per month & adults receive six. Visits may be conducted as individual or group sessions. Direct Service Workers (DSW) are required to collaborate with clients therapists once per month to coordinate services. Health Home Health Home provides a holistic approach to improving clients physical & mental well-being. Any client who is enrolled in PRP is eligible for HH, whether they have a chronic medical condition and/or experience difficulty managing their mental health condition. LBE has a Registered Nurse & Nurse Practitioner on staff who create health wellness goals for clients, coordinate care with PCPs, facilitate on-site group sessions, & make referrals to specialists such as nutritionists. The DSWs provide one face-to-face service per month to educate clients about their health & can also assist with tasks such as scheduling medical appointments. Therapeutic Behavioral Services TBS is an intensive, short-term, in-home, behavior modification service. It is offered to minors who exhibit destructive behaviors that put them at risk for out-of-home placement. Clients are authorized for approximately 5-25 service hours per week based on individual needs. Caretakers are required to be present & participate in visits as the goal is to transfer effective behavior management skills to the guardians.
