- The Oregon Patient Safety Commission is a semi-independent state agency created by the Oregon legislature in 2003. Our mission is to improve patient safety by reducing the risk of serious adverse events occurring in Oregon's healthcare system and by encouraging a culture of patient safety through a variety of patient safety programs.VisionSafe healthcare for all Oregonians.MissionTo Improve patient safety by reducing the risk of serious adverse events occurring in Oregons health care system and by encouraging a culture of patient safety. To accomplish its mission, the Commission: Operates a confidential, voluntary serious adverse event reporting system in Oregon Promotes quality improvement techniques to reduce system errors Shares evidence-based prevention practices to improve patient outcomesAdministers a voluntary communication and resolution process for patients and healthcare facilities and/or providers when a patient unexpectedly experiences serious physical injury or death as a result of their medical care rather than their illness
