- Liberty's mission is to bring peace of mind to our clients, their families and our community partners by providing extraordinary services through personal care. Services Provided Personal Health Advocacy - This service offers care coordination to assist clients in obtaining and accessing exceptional medical care. Liberty will work to assist all of our clients through improved communication and education to understand the directives of there Care Practitioners and to remove their barriers to improved care. Our trained Personal Health Advocates attend medical appointments with our clients to best understand the directives of the PCP and to assist through education of the client, their families and their support systems to adhere to the directives of the Practitioner. We will work to coordinate necessary services to increase the potential for success of the client. Personal Health Advocacy is a private pay service. Transitional Care Management - By contracting with hospitals and Physicians, our skilled clinical professionals are able to assist people who have been hospitalized to experience a safe and successful transition home. This service includes transportation, coordination of any follow-up appointments, medications reconciliation and education as well as documentation that is provided to the Primary Care Physician and the discharging facility. Our fees for TCM services are 100% reimbursable by the Center for Medicare and Medicaid. Homemaker Services - Homemaker services assist individuals in living independently in their own home. Plans of Care vary from Essential Care to Optimum Care. All services provided are Non-Medical. Services include Care Coordination, Companionship, Meal Preparation, Transportation, Wellness Promotion, Housekeeping, Laundry, Pet Care, Errands, Light Gardening, Organization and Down-Sizing, Medical Appointment Attendance, Physical and Cognitive Exercise plans.
