• Envision Health Partners was established in February 2014 by healthcare professionals who wanted to do more for their patients than just treat injury and illness. With a specific mindset and incomparable design, the members joined together to create a healthcare union that does more than just meet the demands of todays healthcare settings. Their philosophy of care embraces whole-person wellness by combining physical, mental, emotional, spiritual and behavioral areas of health with fitness, nutrition, education, socialization and environmental care & safety. Having over 100 years of combined healthcare experience in a variety of settings has allowed the partners to diversify their programs to meet the current needs of todays senior population by utilizing the continuum of care to optimize treatment plans and personalized wellness paths. Each client receives additional testing and goal-setting beyond what is required by Federal & State regulations, or traditional Medicare A&B benefits and insurance standards. By combining insurance reimbursement with modest private pay pricing, Envision has the potential to exceed State & National outcomes through interdisciplinary care, risk mitigation via wellness screenings & on-sight resourcing for efficient response time. Our goal is to provide ongoing support and encouragement regardless of barriers placed by insurance companies and long-term care policies. Through skilled home health services, outpatient therapy, and a comprehensive wellness program, Envision aspires to not only make a difference in the lives of their clients and families, but also provide additional benefits to their community by operating within the healthcare design as it is intended, thus controlling health spending and allocating resources appropriately.
