• Community Link, Inc. (CLI) is a state certified Managed Care Organization designed to offer the Family Care program to eligible elders and adults with disabilities. CLI employees coordinate the provision of services and benefits available under Family Care to eligible adults with disabilities and frail elders, often with a secondary diagnosis of mental illness. We are here to help members connect to the resources they need to lead fulfilling lives in their communities. CLI works with each individual to learn: - Where they want to live - The relationship they have with the people that care about them - How they want to spend their days - Activities that are important to them - How they are involved in the community - Options for the best possible health outcomes - Supports and resources that are important - Options for how to get to and from places in the community With this information, a plan is designed to meet the unique and specific needs of each member.
