- Rehab Care Coordination is a care management company with over 25 years of experience in providing life care planning and case management services to clients (children, adults & families) with traumatic brain, spinal cord, and orthopedic injuries as well as mental illness. We were the first company in San Diego to specialize in Case Management and Life Care Planning. Since then we've managed the care of hundreds of patients throughout California as well as written life care plans and testified in 12 states. Our staff, with a combined 150 years of hands on experience in the medical community, will be your partner, coordinator, advocate, navigator and cheerleader as we help you reach the highest level possible. We believe our work is not done just having a patient go from appointment to appointment. Our goal is not to just see you attend appointments and rehab but to help you find something to live for. Numerous of our patients have been with us from childhood into adulthood and reached potentials they never thought possible when they first came to us for help. From the moment of injury to when you return home and beyond, we will be there to provide you all the assistance you need.
