• Integra Service Connect helps health plans and providers by ensuring people with health problems get connected with healthcare, community & social services they need. We specialize in finding and engaging disadvantaged, underserved, and high-risk members.Our reputation of understanding and addressing the social determinants of health allows us to overcome key behavioral and social hurdles that impact care outcomes. A core feature underlying all of our services is our feet on the street service model. Integra deploys a virtual workforce of specially trained non-clinical staff Community Coordinators into communities to find and engage hard-to-reach members. Like community health workers or patient navigators, our Community Coordinators identify and address financial, transportation, housing, behavioral, and other barriers these members encounter in accessing needed care and services. Community Coordinators are our front line point of contact, working on the streets every day, locating, engaging, and connecting members to medical, behavioral, social, and community services. Ultimately, they empower members to be engaged in their own health.With existing operations throughout the country, Integra ServiceConnect has worked in the community serving individuals with complex healthcare needs since 1990, under the mission of Better Health Outcomes through the Power of Relationships.
