Direct Access Healthcare

• We are dedicated to improving access to healthcare in the primary care setting and transforming the way patients manage long term conditions. We work alongside providers and commissioners to increase access to primary care services including phlebotomy, point of care diagnostics and nurse led health monitoring clinics. NHS services are facing increasing pressures caused by an ageing population and multiple long-term chronic health conditions. In addition, social care, mental health and community services are underfunded, leaving patients feeling let down and frustrated. By increasing access to direct care, patients can manage their long term chronic conditions at home or with the support of a community pharmacy, reducing hospital admissions. Current projects include: - Increasing Phlebotomy clinics in the evening and weekends - Promoting Local Healthy Living activities and patient mentoring - Increasing patient uptake of NHS Health Checks - Point of Care Testing - Reducing Unnecessary A & E attendance Direct Access Healthcare engage with local commissioners, care providers and patients to develop pathways that strengthen the primary care infrastructure. We promote the adoption of Digital Health Applications and Healthy Living Programmes. With an established network of healthcare professionals, we are a valued partner for commissioners and providers who are developing new approaches to care and expanding systems that already work.
