We offer several programs to meet a broad range of needs:Chronic Care Management Program (CCM) In CCM, our doctors become the patients primary care doctor and directly provide and manage care right in the patients home. Individuals who qualify for the CCM Program are referred to Alegis Care by their health plan after consulting with the original primary care doctor.Complex Care Program (CCP)In CCP, Alegis Care nurse practitioners work as an extension of the patients primary care doctor to carry out customized physician-directed care plans right in the patients home. Individuals who qualify for the Complex Care Program are referred to Alegis Care by their doctor, per the recommendation of the health plan.Health Risk Assessments Program (HRA)Our HRA program offers physicians an in-home solution to increase completion of various health assessments and preventive screenings, with our health professionals acting as an extension of PCPs in the home. We are able to observe and help identify gaps in health care, as well as providing data for coding and HEDIS purposes.
