• Medical Home Network (MHN) is an integrated network of safety net providers that virtually links 14 hospitals and 110 primary care sites on Chicagos south and southwest sides to improve healthcare delivery to vulnerable Medicaid beneficiaries. Established in 2009 and initially funded by the Comer Science and Education Foundation, MHN is transforming healthcare delivery by improving care coordination and health status, increasing access to services, and decreasing costs and the fragmentation of healthcare services. Since launching, MHN has developed MHNConnect, a secure web-based portal, that virtually links it provider participants and tracks patients hospital and emergency department (ED) activity throughout the MHN community, provides real-time patient hospital and ED activity alerts as well as access to pertinent information at the point of care, including Medicaid claims and medication history. As the cornerstone of the areas safety net, MHN aims to not only restructure the way health care is delivered and financed for 170,000 current Medicaid beneficiaries, but also to rapidly impact expansion populations, including individuals who will become Medicaid eligible in 2014.
