• The New York City Health and Hospitals Corporation (HHC) is a $6.7 billion integrated healthcare delivery system with its own 430,000 member health plan, MetroPlus, and is the largest municipal healthcare organization in the country. OneCity Health Services is a newly-created subsidiary of HHC, and its function is to provide strategic, operational, technology, and information technology support to a partnership consisting of HHC and 200 Medicaid provider and other services organizations throughout NYC to achieve healthcare transformation goals under the NYS Department of Health (DOH) Delivery System Reform Incentive Payment (DSRIP) Program. The DSRIP Program is an $8B Medicaid program intended to redesign care delivery to improve outcomes and lower healthcare costs. For HHC and its partners, the potential value of this program is $1.2B over five (5) years. OneCity Health comprises four operating hubs, in Brooklyn, the Bronx, Queens and Manhattan (boroughs where OneCity Health partners provide a full continuum of services), a committee-based governance structure, and a central services organization providing operational management, called OneCity Health Services. To ensure appropriate care for all NYC residents with a focus on preventive, primary care the OneCity Health partnership includes hundreds of community-based healthcare providers, services, and organizations, as well as HHCs network of acute care hospitals, nursing homes, community clinics, home-care service, and MetroPlus, HHCs health insurance plan. Focused on identifying and engaging patients in care before they become sick, OneCity Health is undertaking 11 initiatives, or projects, to become an integrated delivery system of health and social service providers that closes critical gaps in the continuum of care and reduces avoidable hospital use by 25 percent by 2020.
