• Central Wyoming Hospice & Transitions is a non-profit organization that provides the gold standard of end-of-life care to patients and their families. We are the team that you call when you have been given a terminal diagnosis and want to live out the last months of your life on your own terms as you and your family decide together. Central Wyoming Hospice & Transitions is not where you come to die but a service that helps bring quality to your last months -- pain-free, creating memories with those you love, fulfilling bucket list adventures and saying goodbye in a dignified manner. We support you through the journey. You and your family will learn what to expect and how to handle each phase. Most importantly, you will have someone alongside you, all the way. We focus on caring, not curing. And in most cases, we provide care wherever you call home whether its your own residence, an assisted living or nursing home or the home of a family member. We also have two residential homes for patients who may not be able to remain in their own home. Our community volunteers are specially trained to assist patients and their families. They can provide an extra hand when you need it, companionship to the patient when you cant be there or assist with small errands that might make your life easier. If your disease prognosis is more than six months, our Transitions Program will provide a case manager and volunteer support to help you remain as independent as possible, living in your own home. We offer bereavement services to the patient and their family. We offer support groups, individual counseling and a Kids Grief Camp. Central Wyoming Hospice & Transitions is a service available to anyone in the community. Were a team that brings comfort to families when they dont know where to turn. And we bring compassion to our patients and their caregivers when they need us the most.
