- The first commercial provider-owned health plan in New York State, CareConnect was created by Northwell Health (formerly North Shore-LIJ Health System) to make it easy for people to get and stay healthy. Its innovative model is designed to provide access to care thats both excellent and affordable. CareConnect was founded in 2013 and offers a variety of plans for individuals, families and businesses. CareConnects network has grown to more than 20,000 providers at hospitals and physician practices throughout downstate New York. CareConnect members have access to North Shore-LIJ Health System, including Lenox Hill Hospital, the Lenox Hill HealthPlex and Staten Island University Hospital (North and South) as well as Maimonides Medical Center, Montefiore Health System, CareMount Medical, Northern Westchester Hospital, Phelps Memorial Hospital, Wyckoff Heights Medical Center, and the eleven hospitals of the NYC Health + Hospitals.CareConnects provider network also includes Allied Pediatrics, CityMD/Premier Care and all affiliated urgent care centers, ProHEALTH Associates and UPG. MinuteClinic is also part of CareConnects network, providing access to more than 900 walk-in medical clinics in select CVS/pharmacy stores across the country.
